Common Pharmacomistakes in Advanced Illness

Opioid conversion calculations and titrations are a leading cause of opioid dosing errors. This may result in the under-treatment of pain or the excessive use of opioids – and potentially the development of adverse effects.

In this presentation, participants will learn to detect an error “in the making,” describe the rationale for what was done incorrectly, and identify a preferred approach to the problem at hand. We will examine best practices in choosing candidates for opioid prescribing, titrating continuous infusions of opioids, transitioning to and from transdermal fentanyl, dosing methadone and more. Like insulin, opioids can be unforgiving when an error is committed. It’s preferable to avoid the error altogether!
